The MSF published Safety Flash 12-07 regarding an incident in which, during a ‘routine’ cargo discharge operation, the corner stacking point on a skip snagged on the top of one of the vessel’s safe haven openings.
What happened?
This resulted in a 21 tonne overload on the platform’s crane and damage to the skips lifting bridle and stacking point.
